A physician&#8217;s assistant will be scheduled multiple times throughout the year for any athlete to complete their physical on campus.\u00a0 See Coach\u00a0<span class=\"markhwquu8aau\" data-markjs=\"true\" data-ogac=\"\" data-ogab=\"\" data-ogsc=\"\" data-ogsb=\"\">Sherwin<\/span>\u00a0for details and dates.\u00a0 ($20)<\/div>\n<div class=\"x_elementToProof\">Sports physicals facts:<\/div>\n<ul>\n<li>Please make sure your sports physical gets you through the entire sports season\/school year;<\/li>\n<li>Sports physicals do NOT have to be completed at your primary care facility or through your primary care physician;<\/li>\n<li>Sports physicals can be obtained through any MD, DO, APRN or PA;<\/li>\n<li>Sports physicals can be completed at any quick care facility (i.e. Velocity on Youree, now called Trinity Clinic, WK Quick Care on Ellerbe) on a walk in basis for $35;<\/li>\n<li>\n<div class=\"x_elementToProof\" role=\"presentation\">Hours of operation vary (please contact that facility directly).<\/div>\n<\/li>\n<\/ul>\n<div class=\"x_elementToProof\">Take this form:\u00a0<a id=\"OWA0f6592eb-7faa-7a92-1bb5-c909ce8037df\" class=\"x_OWAAutoLink\" title=\"https:\/\/caddomagnet.net\/magnet\/wp-content\/uploads\/2023\/07\/MedicalHistoryMedicalExam.pdf\" href=\"https:\/\/caddomagnet.net\/magnet\/wp-content\/uploads\/2023\/07\/MedicalHistoryMedicalExam.pdf\" target=\"_blank\" rel=\"noopener noreferrer\" data-auth=\"NotApplicable\" data-linkindex=\"0\">LHSAA MEDICAL HISTORY EVALUATION<\/a>\u00a0 to your physician.<\/div>\n<div class=\"x_elementToProof\">Before going to the physician, parents will complete the\u00a0<b>Medical History<\/b>\u00a0and\u00a0<b>Parental Waiver<\/b>\u00a0sections COMPLETELY with all questions answered, dates made, signatures and boxes checked.\u00a0 These are the first two sections of the physical.<\/div>\n<div class=\"x_elementToProof\">The physician will complete the\u00a0<b>medical evaluation<\/b>, clear the athlete for sports, sign and date the last portion.<\/div>\n<div class=\"x_elementToProof\">Once completed, upload the sports physical on Aktivate and a copy of your birth certificate on Aktivate.<\/div>\n<div class=\"x_elementToProof\">BOTH DOCUMENTS need to be scanned with\u00a0<span style=\"color: #ff0000;\"><b>proper orientation<\/b><\/span>, so they are legible by both the athletic director and LHSAA auditor.\u00a0 If they are not properly scanned and uploaded, they will be rejected.<\/div>\n<div class=\"x_elementToProof\">In Aktivate,\u00a0 complete all online questions, forms and signatures. The athletic director will review all uploaded documents and will upload the students&#8217; transcripts and report cards. If it says \u201cADMIN\u201d that is not something the parent needs to complete, that is something the athletic director will complete. Please note, Fall Report Cards will not be uploaded until after the fall semester is completed. Please read all of the announcements on Aktivate.<\/div>\n<div class=\"x_elementToProof\"><\/div>\n<div class=\"x_elementToProof\">**If you are not an LHSAA sport: archery, lacrosse, fencing, dance, E-sports, cheer, PLEASE see your coach directly for your specific requirements which may be slightly different.**<\/div>\n<div class=\"x_elementToProof\"><\/div>\n<p>Take the LHSAA Medical History Evaluation form with your and have the top and middle portion COMPLETELY filled out prior to obtaining a sports physical \u2013 <a href=\"https:\/\/caddomagnet.net\/magnet\/wp-content\/uploads\/2023\/07\/MedicalHistoryMedicalExam.pdf\" target=\"_blank\" rel=\"noopener\">LHSAA MEDICAL HISTORY EVALUATION<\/a>\u00a0 Once completed, upload a copy of the sports physical on Aktivate and upload a copy of your birth certificate on Aktivate. Complete all online questions, forms and signatures. The athletic director will review all uploaded documents, and will upload the students transcripts and report cards. If it says &#8220;ADMIN&#8221; that is not something the parent needs to complete, that is something the athletic director will complete.
