You can also see your counselor in the Counselor\u2019s Complex. Please complete the Record Request Form below and submit it to Mrs. Bond. For <strong>current students<\/strong>, payment may be submitted through your <a href=\"https:\/\/www.payforit.net\/MakeItemPaymentEx.aspx\">PayForIt<\/a> account.<\/p>\n<p>Additional instructions are included on the Record Request Form.<\/p>\n<p><a href=\"http:\/\/caddomagnet.net\/magnet\/wp-content\/uploads\/2015\/01\/Transcript-ACT-Score-Report-Request-Form-updated-2016.pdf\">Transcript &amp; ACT Score Report Request Form &#8211; updated 2016<\/a><\/p>\n<p><span style=\"text-decoration: underline;\"><strong>ALUMNI Transcript Requests<\/strong><\/span><\/p>\n<p>1.For<strong> graduates<\/strong>, you will use <a href=\"https:\/\/www.locallevelevents.com\/users\/login\">Local Level Events.<\/a><\/p>\n<p>2. At the home page, click on &#8220;Find Events,&#8221; then enter zip code 71101. This will bring you to the correct page.<\/p>\n<p>7. Once your order has been processed, please email payment confirmation to Mrs. Bond, at <a href=\"mailto:kbond@caddoschools.org\">kbond@caddoschools.org<\/a>. Should you have questions, Ms. Bond may be reached at 318-364-5067. Please allow ONE WEEK (5 working days) for the processing of your request.<\/p>\n<p>*Note \u2013 ACT score reports are NOT available for alumni.
